Where does the .htaccess file go?

In the directory it should apply to, usually the site root. It applies to that directory and everything beneath it.

Which status code should a permanent move use?

301. It tells browsers and search engines the move is permanent so they update to the new URL. Use 302 only when you genuinely intend to move back.

My rules do nothing. Why?

Apache must have AllowOverride enabled for the directory and mod_rewrite loaded. On nginx, .htaccess is ignored entirely — it is an Apache-only mechanism.
